Release of Liability, Waiver & Consent

1. RELEASE OF LIABILITY & WAIVER

In consideration of PeakForce Hockey Development Inc., a corporation incorporated under the laws of Newfoundland and Labrador (the "Company"), permitting the individual named below as participant (the "Participant"), being my minor child or ward, to participate in hockey training camp provided by the Company (the "Activities") at the facilities designated by the Company (the "Property"), and for other good and valuable consideration, I agree to all the terms and conditions set forth in this release of liability, waiver, and assumption of risks (this "Agreement").

I hereby expressly waive and release any and all claims which I have or may in the future have against the Company, its affiliates, rink owners, and their respective directors, officers, shareholders, partners, employees, contractors, staff, coaches, trainers, agents, representatives, successors, and assigns (collectively, "Releasees"), on account of injury, illness, death, and/or property damage arising out of or in any way related to the Activities and/or the Participants participation therein, due to any cause whatsoever, including, without limitation, to the extent permitted by law, the negligence (but excluding gross negligence or willful misconduct), and/or the breach of contract and/or the breach of any statutory and/or other duty of care owing under applicable legislation or otherwise by, the Company and/or any other Releasees. I covenant not to make or bring any such claim against the Company and/or any of the other Releasees, and forever release and discharge the Company and all other Releasees from liability under such claims.

2. INDEMNIFICATION & MEDICAL AUTHORIZATION

I shall defend, indemnify, and hold harmless the Company and all other Releasees against any and all losses, damages, liabilities, deficiencies, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including, without limitation, reasonable legal fees, against or which may be suffered or incurred by the Company and/or the other Releasees arising out of or in any way related to the Activities and/or participation therein by the Participant, except to the extent caused by the gross negligence or willful misconduct of the Company and/or any other Releasees.

I acknowledge that the Participant has no medical condition(s) which could put the Participant at an increased risk of injury during the Activities. I authorize the Company and its representatives, employees, contractors, coaches, and other staff to consent to emergency medical treatment on behalf of the Participant.

3. SOCIAL MEDIA / PHOTO CONSENT

I grant permission to PeakForce Hockey Development Inc. (the "Company") to use images of my participating child.

Such use includes the display, distribution, publication, transmission or otherwise use of photographs, images and/or videos taken for use in materials that include, but may not be limited to, printed materials such as brochures, newsletters, videos and digital images used on the Company’s website and social media pages. This permission is granted for promotional and marketing purposes related to the Company's hockey development programs and services. The Company will not use such images in any manner that is defamatory, unlawful, or that would violate the minor's privacy rights beyond reasonable promotional use.
